I/{first_name} hereby understand and acknowledge that the training, programs, and events held by A1A BEACH CLUB may expose me to many inherent risks, including accidents, injury, illness, or even death.
I/We assume all risk of injuries associated with participation including, but not limited to, falls, contact with other participants, the effects of the weather, including high heat and/or humidity, and all other such risks being known and appreciated by me.
I/We acknowledge that I am physically fit and mentally capable of performing the physical activity I choose to participate in. After having read this waiver and knowing these facts, and in consideration of the acceptance of my participation and the A1A BEACH CLUB furnishing services to me, I agree, for myself and anyone entitled to act on my behalf, to HOLD HARMLESS, WAIVE, AND RELEASE A1A Beach Club, its professors, management, staff, employees, organizers, representatives, and fellow students from any responsibility, liabilities, demands, or claims resulting from personal injury or loss of personal belongings, and any likelihood of contracting COVID-19 related to of any kind arising out of my participation in the Fight Sports Club training, programs and/or events.
I/ also hereby state that students named above are physically fit to take the prescribed course of instruction and do so of their own free will for an agreed-upon fee. I understand there is no refund policy on any payment to this academy.
By entering A1A Beach Club, I acknowledge I am entering an area where photography, audio, and video recording may occur. My entry and presence on the premises constitute my consent to be photographed, filmed, and/or otherwise recorded and to the release, publication, exhibition, or reproduction of any and all recorded media of my appearance, voice, and name for any purpose whatsoever in perpetuity in connection with A1A Beach Club and its initiatives, including, by way of example only, use on websites, in social media, news, and advertising. By entering the premises, I waive and release any claims you may have related to the use of recorded media of me at the event, including, without limitation, any right to inspect or approve the photo, video, or audio recording of you, any claims for invasion of privacy, violation of the right of publicity, defamation, and copyright infringement or for any fees for use of such record media. I understand that all photography, filming, and/or recording will be done in reliance on this consent.
By my signature I/We indicate that I/We have read and understand this Waiver of Liability. I am aware that this is a waiver and a release of liability and I voluntarily agree to its terms.
How To Cancel
The membership may be cancelled at any time with 30 day written notice by the Member to A1A Beach Club, at which time final Monthly Membership Dues will be deducted from the bank account of the Member or billed to the credit card account of the Member. All cancellation requests are accepted at the gym only or through the phone to confirm that membership is canceled.
20% of your membership will be billed for cancellation fee
Fees
Cancelation fee will be added to your account to freeze or cancel. The fee will be 20% of total membership.
2. I understand that monthly dues will be billed on the 1st of each month and shall be billed to the credit card used for payment today
BY AGREEING TO THE TERMS ABOVE, THE MEMBER WARRANTS THAT HE/SHE HAS READ, UNDERSTANDS, AND AGREES TO ALL CONDITIONS OF THIS MEMBERSHIP AGREEMENT.
